Installment Plan Agreement*

Buyer agrees to purchase the Patented Back Builder in four equal payments of $80.74 (plus 7.25% Illinois residents) to be charged monthly.  Buyer authorizes Back Builder, Inc. to charge the credit card (MasterCard, Visa, Discover, American Express) of buyer's choice at one month intervals for four months.  Back Builder, Inc. agrees to ship the unit within three days of receipt of completed agreement and credit information or as specified in availability notice on web site.**  Should buyer decide to return the product within 30 days, Back Builder, Inc. agrees to refund the charged amount minus $19.95 shipping and handling.
